The purpose of this test is to determine whether light bends correctly when it passes through your lens or if you have a refractive error, such as nearsightedness. During the refraction test, you’ll use a device with different lenses to view an eye chart 20 feet away.

Steps
- INITIAL BEST SPHERE CORRECTION. A patient without astigmatism should be fully corrected after this step alone.
- SEARCH FOR Astigmatism. The Jackson Cross Cylinder (JCC) is used in this step.

- REFINE CYLINDER POWER.
- ADJUST FOR BEST SPHERE CORRECTION.
What does a Phoropter do?
A phoropter is used to manually determine “refraction” – exactly how a lens must be shaped and curved to correct your vision to a normal state, nothing more. Phoropters are subjective however, based on your visual perception and response to your eye doctor’s questions.
What causes refractive surprise?
The presence of corneal pathology such as corneal scarring or oedema can influence the refractive outcome. Post-operative cystoid macular oedema can cause a hyperopic shift.

The goal of the subjective refraction is to achieve clear and comfortable binocular vision. The clinician’s ability to maintain control during the refraction is directly related to their ability to communicate clearly with the patient.
How do you balance binoculars?
Binocular Balance Binocular balancing can be done using the Risley prism on the phoropter or by alternate occlusion. In either case, you should start the binocular balancing procedures by adding +0.75D sphere to both eyes so that the patient’s visual acuity is blurred to the 20/30 – 20/40 level.
